About referring yourself to Bury Live Well Service
Our Live Well Service encourages and supports self-referrals.
To help us understand your current situation we need you to answer some questions about yourself and your health and wellbeing and let us know what type of support you think you may benefit from. This allows us to make sure we can offer you the support that is right for you.
If you choose to refer yourself, or make a supported referral, we will ask if we can notify your GP of your referral. We will also ask you for your National Health Service (NHS) number. This is so that we can keep your GP fully up-to-date with your health and wellbeing and so that health professionals can easily find your health records. You do not have to give your consent for us to notify your GP if you prefer not to. You do not have to provide your NHS number if you don't know this or you prefer not to.
Any information you give will be held securely and treated in the strictest confidence, in accordance with Bury Council's Live Well Service privacy policy.
How to get help
There are three ways you can request support from our Live Well Service:
- Make a self-referral
- Make a supported self-referral (when you get help from someone else to make your referral, eg a friend or family member)
- Ask your GP, health practitioner or health specialist to make a referral on your behalf
If you are aged 16 or under, you must be referred by your parent or guardian, or by your GP or health specialist.

The next steps
After we receive your completed referral form we will be in touch within five working days of us receiving your form. If we cannot contact you by phone, we will send you an email or a letter.
After discussing your referral with you, if the support we offer is right for you, we'll arrange a time, date and venue to meet up and welcome you to our Live Well programme.
About referral via a GP, health practitioner or health specialist
If you prefer to be referred to our Live Well Service by your GP, health practitioner or health specialist, please discuss this with them at your next appointment.
