Northern Care Alliance Children's Community Nursing
The Children’s Community Nursing Team consists of Registered Children‘s Nurses with hospital and community experience. The team structure comprises of a Team Lead, a Specialist Diabetes Nurse, a Complex Needs Nurse, Senior Nurses, Staff Nurses, Health Care Support Worker and an Administrator.
They work with parents/carers within the Bury area to provide nursing care, support and health education for Children and Young People aged 0 to 18 years (19 years if complex, in full time education or under a paediatric consultant)
They aim to ensure the child or young person can receive nursing care and treatment in the community setting for both acute and chronic conditions which can be delivered either at home or in one of our children’s clinics dependent on their needs.
Community Paediatric Services and child health
Bury community paediatric medical team provides consultant lead services for children registered with Bury GPs. The entire medical team is based at Fairfield General Hospital (FGH).
The medical team undertakes clinics in a variety of settings e.g. in paediatric outpatient clinics, Child Development Centre, Paediatric Audiology suite –all situated in FGH. The team also undertakes clinics in Bury’s special schools, community clinics in Prestwich, Tottington, Radcliffe or Bury Living Well Centre.
The medical team works closely with all levels of hospital and community based children’s nursing staff, children’s Allied Health Professionals (AHPS), Paediatric Audiology Staff, Bury Public Health department, Bury Education department including teachers, SEND-CO, SEND-Team and Bury’s Social Care service (Children’s Disability Team, Adoption and Fostering Team and children’s safeguarding hub). The medical team also works closely with voluntary agencies e.g. Bury2gether. The medical team consists of consultants, associate specialists and junior and senior Trainees in Community paediatrics.

Child and Adolescent Mental Health Services (CAMHS).
Bury CAMHS (child and adolescent mental health service), formerly Healthy Young Minds, offer specialist services to children and young people who are experiencing mental health difficulties. They help children and young people up to the age of 16 years, and provide assessments, intervention and support to their families/carers.
The community dietetic service provides nutritional assessments and advice on appropriate diets for specific clinical conditions to adults and children, carers/relatives and staff.
Our aim is to help and support patients to follow their diet in a manageable and practical way.

Cambeck Close Short Break Service :: Pennine Care NHS Foundation Trust
Cambeck Close provides overnight Short Breaks and support to learning disabled children and adults with additional healthcare needs.
Our team consists of registered nurses, assistant practitioners and support workers and we operate from two purpose-built, accessible bungalows in Whitefield, Bury.

Children’s community learning disability team, Bury :: Pennine Care NHS Foundation Trust
Bury Children's Learning Disability Team
The team is provided by Pennine Care NHS Foundation Trust.
We support children with a moderate to profound level of Learning Disability, their families and carers and who are registered with a GP in the Bury area.
Our team consists of three Learning Disability nurses.
All interventions are based upon the following assessments: Comprehensive behaviour assessments, health assessments and risk assessments.
We offer support to other services around individual children, including: advice on health, behavioural and care issues, individual behaviour support and training, parenting advice and family work and training for staff and carers.

Referral for Oral Care to the Community Dental Service
In Heywood, Middleton and Rochdale, Bury and Oldham Referrals are accepted for children with additional care requirements or that require specialist care that is unable to be managed by a General Dental Practitioner (GDP).
· Anxious children unable to cope in a General Dental Practice, or those patients requiring inhalation sedation.
· Children who require specialist dental management, e.g. defective teeth, missing teeth or abnormally formed teeth, cleft lip and palate, dental trauma, oral pathology.
Referrals can be made only by non-dental professionals; your referral will be returned to you if sufficient information is not supplied. Please see downloadable referral form.
