- Refer to or convene multi-agency meetings to assess persistent or complex needs. Services may include:
- Explore Bury’s local services via the SEND Local Offer and Health Services for SEND young people 16+.
- Develop individualised and personalised programmes of support.
- Support young people to introduce themselves to their GP before transitioning to adult services.
- Encourage young people to take increasing responsibility for their health, in line with the Mental Capacity Act (2005):
- From age 16, young people are presumed to have capacity to make decisions about their healthcare.
- At 18, they legally become adults and transition to adult health services. Planning for this should begin early.
