• Provide individualised programmes to support complex needs and transitions into independent living.
• Deliver specialist interventions around:

• Refer to multi-agency teams for assessment and support, including:
  • Speech and Language Therapy;
  • Child and Adolescent Mental Health Service;
  • Occupational Therapy Service;
  • Physiotherapy Service.

• Use specific resources such as those available on the Down’s Syndrome Association or the Autism Central.
