  • Use tools such as the Vocational Profile Workbook to help young people identify their strengths, interests, and support needs.
  • Incorporate resources from the When I Grow Up Facilitator’s Handbook to guide structured sessions around aspirations and work readiness.
  • Where care needs are anticipated post-education, ensure the SEND Lead supports access to Adult Social Care Transition Team.

  • Deliver independent travel training to build confidence and competence in navigating public transport.
