What is the Exercise Referral Scheme?

The exercise referral scheme is a 12 week programme which helps individuals with long term health conditions set clear goals on how to improve their health, wellbeing and overall quality of life. This can take place at home, outdoors or at a local leisure facility.

Live Well support and advice is free, however, if you decide to use local leisure facilities and attend some of our community sessions there will be a discounted fee.

Who is this exercise referral for?

For people that have:

  • A chronic heart condition
  • Diabetes
  • A mental health condition that is stable
  • Respiratory or pulmonary conditions
  • Neurological conditions (such as Parkinsons or a stroke)
  • Long Covid
  • Been identified as frail or require strength and balance interventions
  • A BMI over 30
  • Are registered with a doctor's surgery in the borough of Bury
  • Over the age of 16

How can I get access?

If you think you may be eligible for a referral, you can make a self referral or ask your health care provider to make a referral on your behalf.

What will happen at the meeting with the Exercise Referral Officer?

During the first meeting your medical and lifestyle history will be reviewed and basic health checks will be carried out. The exercise referral officer will also discuss your exercise preferences and goals. Using the information gathered, a personal exercise programme will be designed for you.

The officer will support you over a 12 week period to achieve the goals set and provide motivation and advice on how to improve your activity levels in a way that is manageable and achievable. You will have regular contact over the 12 weeks with the officer to keep you on track.
