Both these options will take you to information on services. You can find out what a service is, who it is suitable for, how much it costs and how to refer to the service.

Contact Services

If you need a postal address, email address or telephone number for any service within Adult Care you will be able to find them quickly and easily on Your care, Your choice. You can also fill out a contact me form and someone from the service will contact you to discuss your needs.

Refer yourself for a service

If you require universal services such as CareLink or Telecare, you can complete an online referral form. Alternatively, you can fill out your details on a 'Contact Me' form and the service will contact you to make the referral on your behalf.

Tell us your needs

If you don't know what your needs, or friends/relatives needs are, you can complete an online assessment. There are questions about your daily life such as how well do you manage to bathe or prepare meals. Simply choose an option from the drop down list and submit. You will then be contacted and we will tell you how we can help.
