What is a Care Plan?

A personal care plan, based on your identified needs, is drawn up stating what services will be provided, when and by whom.

The Care Plan, of which you will keep a copy, includes practical information on services provided and contact telephone numbers. Within six weeks your Care Plan will be reviewed and following that on a yearly basis. The Care Plan can be altered should your needs change. Click on the link on the right for more information.
