NHS Continuing Healthcare

What is Continuing Healthcare?

NHS Continuing Healthcare (CHC) is a free package of health and social care which is arranged and funded solely by the NHS for individuals who have on-going primary health needs.

You can get CHC funding in any setting including your own home or a care/residential home. You cannot get CHC funding if you are in hospital.

CHC funding is managed and assessed by Bury NHS. It has a dedicated CHC Team.

You or your family can request NHS Bury to carry out a initial assessment to see if you screen in for a full assessment for CHC. To request an initial assessment contact either the CHC Team on 0161- 762-3119/3198 or the Bury Council Customer Contact Center - the number is in the 'contact us' on the top right of this page.

Deadlines for assessment of eligibility for NHS CHC funding for cases during the period 2004-2012

On the 15 March 2012, the Department of Health announced the introduction of deadlines for individuals to request an assessment of eligibility for NHS Continuing Healthcare funding for the period 1 April 2004 to 31 March 2012.

The deadlines apply to previously un-assessed periods of care, which occurred in the following time periods:

Period - 1 April 2004 - 30 September 2007, deadline 30 September 2012.

Period - 1 October  2007 - 31 March 2011, deadline 30 September 2012.

Period - 1 April 2011 - 31 March 2011, deadline  31 March 2013.
