
In March 2003 Bury MBC opened its first integrated extra care scheme which is a joint venture between Adult Care and Six Town Housing. The scheme provides 24hour care and support to 69 tenancies. The care and support is provided by Adult Care Services Home Care Team who are based on site and have a minimum of 2 staff on duty 24 hours a day 7 days a week. Clients are assessed by the Care Team for the levels of Care and Support they require. The Care packages are tailored to each individuals need and are charged for in accordance with Adult Care Services charging policy. The Support is delivered and charged for in one of the following 4 categories:-
Basic - 1 hours per week,
Low - 2 hours per week,
Medium - 4 hours per week,
High - 8.5 hours per week.
What is the objective of the service?
The objectives of Falcon & Griffin Extra Care scheme are to:-
- provide a real alternative to Residential Care for older people.
- to enable Residents to retain control over their life whilst receiving the Support & Care they need in a secure environment.
- to enable Older People to live in their own home & environment.
- to provide an increased range of services.
- to enable Support and Care packages to be tailored to individual needs.
Who is it for?
Extra Care Sheltered Housing is for people over the age of 60. However, younger applicants with disabilities / high support needs may also be considered. In order to apply for Extra Care Sheltered Housing a Housing Application form must be completed.
