Community care means helping people to live as independantly as possible either at home, in the community or in short or long term care. These services are avaliable to people with a wide range of needs including:
- Older people who are frail or vulnerable
- people with a physical disability
- people with a sensory disability (sight or hearing problems)
- people with a learning disability
- people with a mental health problem
- people with a drug or alcohol problem
- people with HIV and aids conditions
These services are available regardless of age, gender, sexual orientation, ethnic or cultural background, disability or health status. Community care services are based on an assessment of needs of both users and carers. Individuals with more complex needs may receive a care management service.
Community Care Services include:
- day care
- home care
- meals on wheels
- equipment to help with daily living
- short break care away from home
- care in residential or nursing home/long term care
- help with interpretation
- signing, minority languages, large print
- Health visitors
- District Nurses
- Self help groups
- Services from voluntary organisations
