What is Step Down Intermediate Care?

Step Down Intermediate Care is a term used to describe specific recovery and/or convalescence care, designed to allow a period of time for older people to recover after a spell in hospital. There may also be a period of rehabilitation after this recovery period.

The service has been set up to improve the chances for a successful discharge from hospital, providing time for older people to recover.

Aims of the scheme

  1. To move away from an unnecessary length of stay in a hospital setting.
  2. To maximise the potential for early recovery.
  3. To maintain and/or develop the skills of daily living.
  4. To provide a facility that adds to the range of services available to promote and improve the health and social care needs of the older people of Bury.

Who is the scheme for?

People who:

  • are 65 years old or more
  • are ordinarily resident in Bury Metro.
  • are registered with a Bury GP
  • are in Fairfield Hospital and are assessed as no longer requiring twenty-four hour medical or nursing care within an acute hospital setting or nursing home.
  • are assessed as having the potential to benefit from a period of recovery and convalescence support with aspects of daily living.
  • consent to come on to the scheme, understand the nature of the services available and that this care is time-limited.

Other features of Step Down Intermediate Care

  • The service is free of charge to the person using the scheme.
  • All medical and nursing support is provided from primary and community services.
  • Throughout the stay, District Nursing Services monitor individual care.
  • The maximum length of stay is six weeks but it is likely that most people will be discharged before that time.
  • On leaving the scheme, suitable services will be identified and arranged if necessary, such as home care or day care or admission to residential care. These may be chargeable.
