Carers Health Project

Every year small numbers of people are forced to stop caring because of their own illnesses or injuries.  Very often this could have been prevented if support, information and guidance had been available.

To assist those patients who are Carers and help prevent potential health issues several GP practices in the Bury area are involved in developing a new health service for Carers. There is now a Health and Social Care Project Worker based at several GP’s practices.

This person will provide support to help Carers, complete referrals and give information about other services that maybe available to Carers. Also, in your GP’s practice there is a Carers Notice Board giving details of Carers Support Groups, new services for Carers and general Carers issues.

The Health and Social Care Project Worker can help you receive services that maybe available to you for example: support from Social Services; benefit advice or services and registering onto a Carers register; so that you receive relevant information quarterly.

The information that you give during this appointment will remain confidential and if you wish to receive any other services, you will be asked to sign a consent form so that your information can be shared with the appropriate service provider.

In addition, you own GP will be made aware that you are a Carer, and will monitor your health in order to avoid any potential health issues.
