Adult Placement Scheme (Shared Lives)
What is an Adult Placement?
Adult placement offers short term breaks (often called respite care) away from home, long term placements for people who need to live in a supported environment and sessional support sessions where day support is offered either in the carers home or out in the community. It is for anyone who is over 18 and who has a disability, including a learning disability. It is a service arranged by Adult Social Care services.
Who can get Adult Placement?
Adult Placement is for anyone over 18 who meet the eligibility criteria who are finding it difficult to manage because of one of the following:
- learning disability;
- physical disability;
- mental health problems;
- problems associated with getting older;
- older people with mental health problems, including dementia.
We aim to help those with the greatest need, and we measure this using certain standards, which we call eligibility criteria. There are four standards we use to measure need:
- People who need a little help to keep them well and in touch with others.
- People, and their carers, who need help to be independent.
- People in crisis whose lives may be at risk but who we may be able to help at home so they can stay independent.
- People in crisis whose lives may be at risk and who may need to go into residential care.
What's different about Adult Placement?
Many adults are looking for somewhere new to live. For example.
- A person may be returning to the community from hostels or long-stay hospitals
- A person might not be able to manage on their own
- Or, may be unable or unwilling to live with their own families
How Adult Placement is provided
Long Term - Long term adult placement provides care in an ordinary family home of someone who has been vetted, trained and approved to provide the correct level of support. You will have your own bedroom and you will be listened to and have your wishes respected. You may like to join in with family life, or prefer to be quieter and more private. We aim to provide the best service for you that best suits your needs.
Short Term - Short term adult placement is also provided in the carers home and can include anything from a one night stay to a number of months. The aims of short term placements are to give respite to their carers (family or friends) and to give the individual a break from their usual routine.
Sessional Support - Sessional support is day care provided by the carers either in their own home or out in the community. This care is flexible and can be anytime during anyday of the week.
