Your health, your care, your say is a major public engagement exercise to find out what people want from community health and social care services.
The findings will be used to support the next stage of reform and improvement in community health and social care services in England.
Your Health Your Care Your Say aims to build on the Green Paper: Independence Well-Being and Choice and by taking part in this consultation you will be contributing to a very important process: helping to shape community health and social care services for the 21st century. Your views, along with those of all the other people taking part in the consultation, will feed into a set of proposals to improve community health and social care services for everyone in England.
This is a real opportunity to have your say and help make community health and social care services better for you, your family and your fellow citizens.
