Learning Disability Support Team
Our aims are to enable people to develop a valued life of their own choosing within their local community. We aim to promote as much independence as possible, so the support we provide is very much based on the individual's needs.
The support we provide
Depends on individual assessment and ranges from short, weekly visits to support whenever the person is at home, including overnight stays.
The nature of the support can be:-
- Practical help
- Emotional support
- Advice
- Moral support
- Skills teaching
We support people with all activities of daily living including:
- looking after themselves
- looking after their home
- accessing services and leisure facilities
- maintaining relationships
We liaise with
- Social Workers
- Voluntary agencies
- Community Nurses
- Day Services
- GPs and Consultants
- Colleges
- Employment agencies
- Psychologists
