The aims of the team are as follows:-
To maximise support to children with disabilities aged 0-18 years and their families in their homes. To provide services to children and families to prevent family breakdown.
The roles of the team are as follows:-
- To respond promptly to requests for service involvement
- To carry out individual assessment of need of eligible children/young people (0-18 years), their family members/carers
- To establish individual care plans
- To identify appropriate service provision in line with the Service's agreed priorities
- To identify appropriate resources in partnership with families and other agencies ensuring that these are co-ordinated and meet their racial, cultural and linguistic needs
- To carry professional responsibility for a caseload
- To participate in the team duty system
- To organise and/or participate in inter-agency planning meetings, care plan reviews
- To maintain case records and provide reports
- To identify any child protection issues
- To observe all statutory regulations, departmental policies and procedures
