The aim of the team is as follows:
To establish and develop a continuum of provision from early years across all phases of education for pupils with physical difficulties and medical conditions in mainstream schools.
The role of the team is as follows:
- Work collaboratively with the Educational Psychologist with responsibility for early years, the Portage Team, Chidren, Extended Schools Team (CEST) and the Child Development Centre to identify those children who will have life long medical needs which may affect their educational provision
- Offer support and advice to parents on the provision available in authority schools and to promote placement within the borough
- In collaboration with the Paediatric Therapy Services, assist support personnel to deliver appropriate therapy programmes for pupils
- Provide advice and training for support staff in the safe principles of moving and handling, environmental assessment and care plans
- Promote good practice in the inclusion of pupils with physical difficulties and medical conditions in all aspects of school life
- Attend statement planning meetings and annual reviews for pupils in mainstream schools with physical and medical needs
- Facilitate the smooth transition of pupils across the phases of education through forward planning and liaison
- In consultation with other professionals assess, provide and monitor the use of appropriate information and communication technology for pupils with physical and medical conditions
- Liaise with families throughout the pupil's school life and, where possible, offer advice on social opportunities in the community
- Work closely with the Planning and Management Services to plan and develop access in authority schools through the efficient deployment of Government funds
- Offer advice to schools on application for special arrangements and special consideration in external examination and Standard Assessment Tests (SATs)
- Assist in the planning and delivery of training for the authority
- Raise awareness of disability issues in schools and promote good practice.
